CLINICAL DATA

Therapeutic indications:
According to the clinical study The Effect of Sildenafil Maleate 10 mg on healing and the evolution of foot ulcers in diabetic patients, it was determined that for Wagner’s ulcers 2 and 3 the vasodilator action on microcirculation resulted in a more effective granulation when compared to patients treated conventionally.

Posology and method of administration:
1 10 mg tablet every day.

Interaction with other medications and other forms of interaction
Sildenafil may potentiate the hypotensive effects of organic nitrates. Drugs that inhibit cytochrome P450 CYP3A4 isoenzyme, such as Cimetidine, Delavirdine, Erythromycin, Itraconazole, and Ketoconazole may reduce phosphodiesterase type 5 inhibitors clearance, so a reduced dose will be required.
Effects on the ability to drive and use machines
It can cause headaches and abnormal vision so it should be used with caution when driving and using machines.

PHARMACOLOGICAL PROPERTIES

Pharmacodynamic properties
Sildenafil is a PDE5 (phosphodiesterase 5) selective inhibitor, which is found in high concentrations in the pulmonary vasculature. Inhibiting PDE5 increases the vasodilatory effects of NO (nitric oxide) and prevents cGMP (cyclic guanosine monophosphate) degradation, this promotes vascular relaxation of smooth muscles and increases blood flow.

Pharmacokinetic properties

• Absorption
Sildenafil is rapidly absorbed after an oral dose, with a bioavailability of approximately 40%. Peak plasma concentrations are reached at 30 and 120 minutes; its absorption rate decreases when taken with meals.

• Distribution
Sildenafil is widely distributed in the tissues and is transported at 96% bound to plasma proteins.

• Biotransformation
It is metabolized in the liver mainly through cytochrome P450 CYP3A4 isoenzymes (main route) and CYP2C9. The largest metabolite, N-desmethylsildenafil (UK-103320) also involves some activity.
The terminal half-lives of sildenafil maleate and N-desmethyl metabolite are of about 24 hours. Sildenafil is excreted in the form of its metabolites, mainly in feces and, to a lesser extent, through urine. Clearance may decrease in the elderly and in patients with hepatic impairment or severe renal impairment.

About safety
Our studies have shown that when taking Icorfin® none of the patients had side effects because of its low molecular weight in the supplied doses (10 mg).
